Online Certificates and Evaluation Now Available
2014 Special Operations Medical Association (SOMA) Conference

Certificate Information: 
 It’s easy as 1, 2, 3!
1. Visit our website: http://cme.uthscsa.edu

2. Click on “CME TRANSCRIPT” (left-hand side of the screen) and follow the instructions to login. If you’ve attended any of our CME activities you already have an account setup – so be sure to login as an existing user using your email address as your username.Use your last name as your password – not case sensitive. 
After you’ve logged in, scroll down to locate the activity you attended.
For physicians: Where there is a certificate icon, please click on the certificate icon – it should bring up a box that allows you to key in the # of credits you are claiming and then print your certificate.  
For other learners: Where there is a certificate icon, please click on the certificate icon – it should bring up a box that allows you to print your certificate. 
 
3. View and print your UTHSCSA CME transcript/certificates where applicable!

The UT Health Science Center San Antonio School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The UT Health Science Center San Antonio School of Medicine designates this live activity up to a maximum of 32.0 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Online Certificates Now Available
Your online certificate is now available. To obtain your certificate, follow the steps below.
Some features may not work properly if using Internet Explorer. For best results please use Google Chrome as your internet browser.
             
1. Click here​, to login.       
 
2. Enter your User Name  (User Name = your complete e-mail address as you stated when you registered)
Enter your Password (Your (temporary) Password = your last name)
*User Name and Password are not case sensitive.
** If you are an existing user and have already modified your password, please use your current password.          
 
3. Once you have logged in, Click on “Evaluation Form”, located on the main page of the screen.
a. Complete the evaluation survey and Click on the "Submit" button at the bottom of the page in order to print your certificate.  
 
4. A new page listing your available certificate will appear.
a. Click on “Print this certificate” for the certificate you have available.
b. A window will open prompting you to enter your total credit/hours attended.
1. Using your Record of Attendance (attached) enter the total credit/hours attended.
2. Click “Ok”.
3. Click “Print”.
4. Click “Done”.
5. On the top left-hand side of the screen, click on “Logout”.
 
If you see a pop-up that says "You cancelled the previous operation"
• Click “OK”
• Click on the information yellow bar that Internet Explorer displays on the top of the browser that says "This website is using a scripted window to ask you for information. If you trust this website, click here to allow scripted windows...”
• Select the option "Temporarily Allow Scripted Windows"
• Try to print the certificate again, this time it will work
